We have our Letter of Authorization and Contract to Ship in two forms for your convenience: as an Adobe Acrobat file (pdf file) or as a web page. If you have Acrobat Reader, the Acrobat file is the better choice for a high quality printout of the form. If you have the latest version of Acrobat Reader, you can fill out the form on your computer, print it, sign it, fax it or mail it to us with a deposit check. If your version of Acrobat Reader is not current, you may need to print the form and fill it in by hand. In any case, we can mail or fax you the form to you if you have any problems with it. You only need to send us the signed second page.
